Evolutionary Healthcare began with a simple yet profound observation: the gap in care between hospitals and skilled nursing facilities was glaring. As dedicated hospitalists, we frequently admitted patients from nursing homes, and it didn’t take long for us to realize that something needed to change.
A Disconnect in Care: Upon reviewing the medical records and charts of these patients, we were struck by the stark difference in the quality of care they received in nursing homes compared to hospitals. Physicians rarely visited their patients in these facilities, and even when they did, they often struggled to remember them or truly understand their unique medical needs. Vital lab results were not consistently reviewed, and the overall management of medical conditions left much to be desired.
A Call for Accountability: Witnessing these shortcomings, we felt a moral and professional obligation to act. We recognized that skilled nursing facilities deserve a higher standard of care and accountability. It was time to bridge the gap between hospital-level care and the specialized needs of patients residing in these facilities.
And so, Evolutionary Healthcare was born.

Chief Executive OfficerDr. Hussien Ballout is the visionary leader and founder of Evolutionary Healthcare, where he drives the organization’s mission to revolutionize patient care in skilled nursing facilities (SNFs). With a deep commitment to integrating technology with innovative healthcare practices, Dr. Ballout has dedicated his career to elevating patient outcomes and transforming the delivery of care in post-acute settings. His leadership at Evolutionary Healthcare reflects his passion for pushing the boundaries of what is possible in patient care.
Dr. Ballout’s journey in medicine began with a strong academic foundation, earning his Doctor of Medicine degree from St. George's University, following a dual major in Biochemistry and Psychology from the University of Michigan, Dearborn. He further refined his clinical expertise during his residency and internship in Internal Medicine at St. John Hospital and Medical Center in Detroit. After years as a hospitalist, Dr. Ballout identified a critical need for improved care in SNFs, which led him to establish Evolutionary Healthcare. Under his leadership, the organization is at the forefront of merging cutting-edge technology with compassionate care, ensuring that SNF residents receive the highest standard of care and improved quality of life.

Chief Operating OfficerCierra Jones is a passionate healthcare leader, driven by her deep connection to the elderly, a value instilled by her grandparents who played a significant role in raising her in Charleston, West Virginia. She holds a Bachelor’s Degree in Sports Science from West Virginia State University, where she excelled both in basketball and golf. Cierra continued her education with a Master’s Degree in Strategic Leadership with a healthcare administration focus from the University of Charleston and furthered her expertise by obtaining a physician practice management certificate through AAOC.
A licensed nursing home administrator, Cierra has held leadership positions including serving as a Senior Executive Director and a Regional Vice President for a Florida skilled nursing facility (SNF) consulting group. She was recognized as part of the Florida Healthcare Association's Florida Leaders Class of 2021 and serves on the legislative committee for FHCA, advocating for improvements in healthcare regulations and policies.
Currently, Cierra’s position as COO, is an integral part of Evolutionary Healthcare's operations, where she plays a critical role in bridging collaboration between providers and facilities, particularly in areas outside of patient care. Her focus is on developing proactive strategies and implementing quality assurance processes to ensure that Evolutionary providers not only deliver exceptional care but also help facilities reduce expenses, increase revenue, and improve overall quality measures.
As a former SNF operator, Cierra understands the importance of looking at the bigger picture—balancing the needs of patients, staff, and families all while ensuring a positive financial performance, making her a well-rounded and impactful leader in the healthcare industry.

Dr. Tamelia Lakraj-Edwards, MD is a skilled Osteopathic Neuromusculoskeletal Physician with a comprehensive background in both pharmacologic and non-pharmacologic care. She earned her undergraduate degree from the University of South Florida and her medical degree from Spartan Health Sciences University, followed by residency training at the Medical College of Georgia and Larkin Community Hospital. With specialized training in both musculoskeletal ultrasound and Osteopathic Manipulative Treatment, Dr. Lakraj-Edwards utilizes advanced imaging techniques and hands-on therapies to enhance diagnosis and treatment strategies for various musculoskeletal conditions. In addition to her clinical work, she has been instrumental in building population health departments across the states of Georgia and Florida, emphasizing her commitment to community health and preventive care. Currently, she serves as adjunct faculty at Burrell College of Osteopathic Medicine and holds a Master of Public Health from Florida International University. Dr. Lakraj-Edwards takes a holistic approach to medicine, empowering patients on their journey to optimal health, whether addressing chronic pain, injury recovery, or preventive care.
Outside of her professional commitments, Dr. Lakraj-Edwards enjoys participating in medical education initiatives and contributing to research in her field. She also values spending quality time with her family, believing in the importance of balance. In her free time, she enjoys engaging in activities such as attending medical conferences, exploring new treatment modalities, and mentoring future physicians, all while creating lasting memories with her loved ones.

Robert Kim, D.O. graduated from Nova Southeastern University College of Osteopathic Medicine and received his medical degree, Doctor of Osteopathy (D.O.). Dr. Kim completed his residency in Family Medicine at Larkin Community Hospital as well as completing a fellowship in Osteopathic Neuromusculoskeletal Medicine (ONMM). After graduating, Dr. Kim has served as assistant faculty to the Larkin ONMM residency program and currently a pain management specialist at various assisted living facilities in South Florida.

Dr. Rashid is a highly experienced nephrologist and internal medicine specialist with a distinguished background in clinical practice and healthcare leadership. Currently, Dr. Rashid serves as the Chief Medical Officer of Pathways Health Partners while also working as an inpatient hospitalist and nephrologist at University of Florida Health Leesburg and Spanish Plaines, where they provide comprehensive care for conditions including chronic kidney disease, ESRD, hypertension, transplant management, and glomerulonephritis. Previously, they held the role of Eastern Regional Medical Director at Monogram Health, overseeing multidisciplinary teams across multiple states and providing telemedicine services to manage complex chronic conditions. Dr. Rashid also founded a Chronic Kidney Disease Program at The Villages Health in Florida and co-founded the Magic Valley Kidney Institute in Idaho, where they served in key leadership roles. With additional experience in hospice care and integrative medicine, Dr. Rashid has consistently demonstrated a commitment to enhancing patient care and advancing health outcomes through innovative approaches in nephrology and internal medicine.

Dr. Kelsie Carrier, D.O. is a dedicated osteopathic neuromusculoskeletal specialist, who places an emphasis on treating each patient as a whole, alleviating pain and improving function through osteopathic manipulation and individualized patient care. With an undergraduate degree in Microbiology and Molecular Biology from the University of Central Florida, Dr. Carrier possesses a deep understanding of the biological sciences, laying the foundation for a career dedicated to integrated, patient-centered care.
Dr. Carrier earned her medical degree from the Georgia Campus of the Philadelphia College of Osteopathic Medicine (PCOM), where a passion and commitment to the osteopathic philosophy of treating the whole person took shape. This holistic approach is implemented into Dr. Carrier’s work, emphasizing the interconnectedness of the body’s systems and the importance of natural healing.
Dr. Carrier completed her specialty training in Osteopathic Neuromusculoskeletal Medicine at Larkin Community Hospital in Hialeah, Florida. Through rigorous training and hands-on experiences, her expertise allows Dr. Carrier to address each patient’s unique needs, empowering them to achieve better health and improved quality of life. In addition to expertise in osteopathic manipulation, Dr. Carrier is skilled in performing ultrasound-guided injections, a minimally invasive technique that enhances precision in treating musculoskeletal conditions and managing pain.
After completing residency training, Dr. Carrier has worked in outpatient office settings, rehab and skilled nursing facilities, using osteopathic manipulation to reduce pain and expedite recovery from a variety of conditions as well as post-operative patients. Dr. Carrier has also served as faculty at Larkin Community Hospital, helping to train and prepare medical students and resident physicians in her specialty.

Jazmine N. Loman, DO is a Board Certified Physician specialized in Family Medicine, Sports Medicine, and Osteopathic Neuromusculoskeletal Medicine. Dr. Loman obtained her medical degree from Philadelphia College of Osteopathic Medicine, Family Medicine Residency training in NJ, and Fellowship training at Larkin Community Hospital in South Florida. Currently Dr. Loman is practicing Osteopathic Neuromusculoskeletal Medicine providing hands-on techniques of Osteopathic Manipulative Treatment and pharmacological Pain Management. Dr. Loman is committed to helping patients improve and regain their mobility, and pain relief.

Katelyn Reismiller PA-C is originally from Anderson, Indiana but has lived in Florida most of her life. She was a graduate of Vero Beach High School in 2006.
Katelyn completed her Bachelors of Science degree from Trevecca Nazarene University in Nashville Tennessee in 2010. She then went on to receive her Masters degree in Medical Science and Physician Assistant Studies from Nova Southeastern University in Fort Lauderdale, Florida graduating with highest honors in 2012.
Katelyn is certified by the National Commission on Certification of Physician Assistants. She has been practicing Internal Medicine in Vero Beach since 2012, and she is honored to be serving in the community in which she was raised.

After Robert Milici graduated from Long Island University in 2016 with a Bachelors Degree in Health Science, he went on to obtain his B.S.N from Seton Hall University. From there, he moved to Florida and began working for the Martin County Cleveland Clinic Hospital System as a Telemetry Floor Nurse.
During his time at the hospital, Robert witnessed the effect Nurse Practitioners had on the patient's lives, motivating him to achieve his M.S.N. and Become a Board-Certified Family Nurse Practitioner in July of 2023. Robert now looks forward to a long career as a mid-level provider.
In his free time, Robert enjoys music and motorsports.

Toya Alder is a highly skilled Advanced Registered Nurse Practitioner (ARNP) with over two decades of experience in healthcare. Her commitment to delivering exceptional patient care and her multidisciplinary approach to health management define her professional journey. Toya holds a Master of Science degree in Adult-Gerontology Nurse Practitioner from South University in Savannah, Georgia, and a Bachelor of Science in Nursing from Western Governors University. She currently serves as an ARNP in Primary Care at Evolutionary Healthcare/Rehabilitation, where she provides specialized care, implements tailored treatment plans, and supports patient recovery initiatives.
Toya's extensive background includes specialized cardiology care at Steward Medical Center and early roles such as a Registered Nurse at Palm Bay Community Hospital. Her career also saw her excel as a Clinical Coordinator at Sebastian River Medical Center, where she managed a 59-bed unit and led process improvement initiatives. Throughout her career, Toya has consistently demonstrated her commitment to patient-centered care, clinical excellence, and continuous improvement, making her a valuable asset in any healthcare setting.
